Enrollment

We are honored that you have considered us to be your childcare provider!

The application process can be started with the forms below. Please download, and fill out completely.

Along with those forms please bring:

A current immunization record and one of the following:

Note from the child’s doctor stating that the child is in good health to attend childcare
The doctor can sign and stamp page 2 of the enrollment. (Physician Statement)
Bring a copy of the child’s last physical. (It must be within one year)

On site you will need to:

Read our Parent Policies and sign acknowledgement.
Fill out a CACFP Meal Benefit Income Eligibility Form.
And select payment options. You need to put a payment option on file. You can pay onsite or with the option to pay online via MyProcare.com.

We will be happy to answer all your questions and if you haven’t received a tour of the facility we will be happy to do so!
